Share buybacks are the cash a company spent during the year repurchasing its own stock, taken from the financing section of the cash-flow statement. Most companies buy back nothing at all — the line is absent rather than zero — so this ranking covers the minority that do. Figures are for fiscal year 2025 and are read directly from each company's SEC filings; the source column on every row links to the document.

HCA Healthcare (HCA) tops the list with $10.1B in share buybacks for fiscal 2025, ahead of Johnson & Johnson ($6.0B) and UnitedHealth Group ($5.5B). The top 10 together report $49.3B in share buybacks; the gap between #1 and #10 is ×3.10. 169 companies in Healthcare report share buybacks, 9.1% of the 1853-filer universe. The last place on the list, Guardant Health, comes in at $45.0M.
